Job Title: Hospital at Home Registered Nurse

Inbound Health is seeking a highly skilled and compassionate Registered Nurse to join our Hospital at Home team. As a Hospital at Home Registered Nurse, you will provide high-quality, patient-centered care to individuals in the comfort of their own homes.

Responsibilities:
  • Conduct thorough nursing assessments and develop individualized care plans to meet the unique needs of each patient.
  • Provide hands-on care, including wound management, medication administration, and vital sign monitoring.
  • Collaborate with patients, families, and healthcare teams to ensure seamless care coordination and transitions.
  • Utilize technology, such as biometric monitoring, to enhance patient care and outcomes.
  • Stay up-to-date with the latest clinical guidelines and best practices in home healthcare.
Requirements:
  • Current RN licensure in good standing.
  • Minimum 1-2 years of experience in a healthcare setting, preferably in home care or acute inpatient care.
  • Strong clinical skills, including assessments, nursing procedures, and care planning.
  • Excellent communication and collaboration skills.
  • Ability to work independently and as part of a team.
