As a Licensed Practical Nurse, you will be responsible for demonstrating knowledge of medications and their correct administration based on the age and clinical condition of the patient. You will also perform patient care considering needs specific to the standard of care for patient's age; includes geriatric and general inpatient rehabilitation patients. Under the supervision of the R.N., you will assist with the assessment on all patients and reassessments as per policy. You will identify and initiate appropriate rehabilitation nursing interventions. You will also carry out the plan of care as indicated by the patient's needs and response to treatment; evaluates overall plan daily for effectiveness; updates interdisciplinary care plan accordingly based on changes in patient's condition and individual needs. You will interact professionally with patient/family and involve patient/family in the formation of the plan of care. You will demonstrate ability to adequately assess and reassess pain. You will utilize appropriate pain management techniques. You will educate the patient and family regarding pain management. You will also perform other duties as assigned. To be successful in this role, you will need to have a graduate of accredited licensed practical/vocational nurse program, current state licensure as Licensed Practical/Vocational Nurse, CPR/BCLS certification, ACLS preferred, IV Certified required or obtained within 6 months highly preferred, and minimum six months' Medical/Surgical experience in an acute care setting preferred. You will also need to have excellent oral and written communication and interpersonal skills. UW Health Rehabilitation Hospital strives to maximize the health, function, and quality of life of those we serve through comprehensive physical medicine and rehabilitation programs. We are accredited by The Joint Commission and CARF-accredited for our Comprehensive Integrated Inpatient Rehabilitation Program and Stroke Specialty Program.
