Job Overview

As a Long-term Care MDS Coordinator at Sheridan Memorial Hospital, you will play a crucial role in ensuring the timely completion of accurate assessments and interdisciplinary care plans for our elderly residents.

Key Responsibilities
  • Complete assessments, Minimum Data Set (MDS) and care plans for all residents within regulatory timeframes.
  • Monitor completion of MDSs by other disciplines and advise team members on incomplete or untimely assessments.
  • Ensure accurate, timely completion of the MDS/RAPs/Triggers sheet for assigned residents.
  • Initiate, monitor and update care plans to result in the best possible outcome for each resident.
  • Generate and distribute a monthly care plan calendar and conduct care plan conferences for each resident and the interdisciplinary team.
Requirements
  • Bachelor's degree in nursing required, with current, unrestricted Registered Nurse license.
  • Minimum two years experience in a supervisory or management capacity, with one year of skilled nursing experience required.
  • Experience with MDS 3.0 and Infection Control preferred, with BLS certification required.
