Job Summary

The Sheridan Memorial Hospital is seeking a skilled MDS Coordinator to join our team. This role requires a highly motivated and organized individual who can assure timely completion of accurate assessments and interdisciplinary care plans for elders.

About the Role
  • Completes assessments, Minimum Data Set (MDS) and care plans for all Elders.
  • Monitors completion of MDSs by other disciplines within time frames prescribed by regulatory guidelines.
  • Advises interdisciplinary team members of incomplete or untimely assessments by disciplines other than nursing.
Key Responsibilities
  • Ensures accurate, timely completion of the MDS/RAPs/Triggers sheet for assigned residents.
  • Initiates, monitors and updates care plans and supporting activities that will result in the best possible outcome for each elder.
  • Generates and distributes a monthly care plan calendar for the following month.
Requirements

This position requires a Bachelor's degree in nursing and current, unrestricted Registered Nurse license. A minimum of two years' experience in a supervisory or management capacity is also required.

Benefits

We offer a competitive salary range of $65,000 - $85,000 per year, depending on experience. Additionally, we provide a comprehensive benefits package, including medical, dental, and vision insurance, as well as 401(k) matching and paid time off.
