LPN - 4 Medical Inpatient Unit Job Description

UnityPoint Health is seeking a skilled LPN to join our team on the 4 Medical Inpatient Unit. As a Licensed Practical Nurse, you will play a vital role in providing high-quality patient care and contributing to the success of our healthcare team.

Responsibilities
  • Collaborate with the healthcare team to deliver patient-centered care, ensuring the best possible outcomes for our patients.
  • Conduct thorough assessments, collect and report data, and document patient information accurately and timely.
  • Administer medications and perform tasks within the scope of your practice, prioritizing patient safety and well-being.
  • Communicate effectively with patients, families, and healthcare team members to ensure seamless transitions in care.
  • Participate in multidisciplinary patient charting, updating plans and goals of care as needed.
  • Assist in educating patients and families on health maintenance, disease prevention, and post-discharge care.
  • Collaborate with the RN and other healthcare team members to achieve patient care outcomes and goals.
Qualifications
  • Graduate of an accredited LPN program.
  • Current LPN licensure in the State of Iowa or compact state.
  • Previous CNA/PCT experience preferred.
  • Strong verbal and written communication skills, with ability to work effectively in a team environment.
  • Proficiency in basic computer skills and ability to learn EMR systems.
