Nurse Practitioner

VNA Health Group is seeking a skilled and compassionate Registered Nurse to join our team as a Home Health Registered Nurse. This is a rewarding opportunity to provide high-quality patient care in the comfort of their own homes.
Job Summary:The Registered Nurse will be responsible for providing comprehensive end-of-life care to patients and their families in a home setting. This includes assessing patient needs, developing and implementing individualized care plans, and providing education and support to patients and their caregivers.
Responsibilities:- Provide direct patient care, including administering medications, wound care, and other treatments as prescribed by physicians.
- Conduct thorough assessments of patients' physical, emotional, and social needs, and develop individualized care plans to address these needs.
- Collaborate with patients, families, and healthcare teams to ensure seamless care coordination and communication.
- Provide education and support to patients and their caregivers on disease management, symptom management, and end-of-life care.
- Document patient progress, including assessments, care plans, and outcomes, in accordance with VNA Health Group's policies and procedures.
- Current RN licensure in the State of New Jersey.
- At least 1 year of experience in hospice or home health care.
- Ability to work independently and as part of a team, with strong communication and interpersonal skills.
- Strong assessment and problem-solving skills, with the ability to think critically and make sound judgments.
- Ability to maintain confidentiality and handle sensitive patient information with discretion.
