The Role
We are seeking a Patient Services Coordinator, Licensed Nurse to join our team. As a key member of our care coordination team, you will serve as a bridge linking patients to the home health care they need. Your primary responsibilities will include:
- Scheduling patient services and coordinating with patients, families, and care team members
- Ensuring that all agency field personnel are properly scheduled to achieve expected levels of productivity and provide timely care
- Providing care to patients in their homes as needed
Requirements
To be successful in this role, you will need:
- Current Vocational/Practical Nurse License, valid state driver's license, and auto liability insurance
- One year of experience as a Licensed Vocational/Practical Nurse in a clinical care setting, with home health experience preferred
- Strong written and oral communication skills, with the ability to thrive in a fast-paced environment
