Job Summary

This position supports the mission of Swedish Medical Center by expanding beyond the traditional focus on medical needs to the needs of the whole person. The Social Worker LCSW is responsible for assessing the psychological, social, cultural, and/or financial needs that impact the high-risk patient's health and recovery.

Key Responsibilities
  • Assess patients' needs and develop individualized plans to address their social, emotional, and financial challenges.
  • Collaborate with healthcare teams to provide comprehensive care and support patients and their families throughout the continuum of care.
  • Develop and implement discharge plans that ensure patients receive the necessary resources and support to achieve optimal health outcomes.
  • Provide counseling and education to patients and their families to promote healthy behaviors and improve overall well-being.
Requirements
  • Licensed Clinical Social Worker (LCSW) in the state of Colorado with no restrictions.
  • Specialty certification in case management preferred.
  • BLS certification through American Heart Association or American Red Cross required.
  • Master of Social Work (MSW) from an accredited school of social work required.
  • Three to five (3-5) years recent clinical social work experience with acute care hospital setting and case management preferred.
