Job Summary

We are seeking a skilled Licensed Practical Nurse to join our team at the Long Island State Veterans Home. As a critical member of our healthcare team, you will provide high-quality nursing care to our residents, working closely with other professionals to ensure timely and coordinated care.

Key Responsibilities
  • Provide nursing care to residents on the nursing unit, adhering to medical and nursing care plans, department policies, and under the supervision of a professional nurse.
  • Monitor residents closely to identify responses to treatments, medication, and changes in health status, providing treatments according to medical and nursing care plans and department policies.
  • Prepare and dispense medication to residents according to medical plans of care and department policies.
  • Assist with residents' activities of daily living, including direct care, observation of status and changes, and supervision of non-professional staff.
  • Document daily in residents' progress notes all pertinent data, information, and observations, and assist with weekly, monthly, and annual documentation descriptive of resident status and care.
  • Communicate immediately all changes and/or incidents to immediate supervisors and complete required documentation.
  • Maintain policies and procedures relevant to medications, including management of controlled substances.
  • Communicate to supervisors, nursing staff, and other departments and disciplines all pertinent information and changes impacting resident care.
  • Listen and respond appropriately to concerns or issues expressed by residents and families.
  • Ensure and provide direct care to residents that is consistent with comprehensive assessments, requests of residents, and goals of plans of care.
  • Work closely with other professionals and disciplines to ensure timely and coordinated care.
  • Participate in the Quality Assurance Program of the Department and serve on committees pertinent to the quality of care of residents.
  • Participate in the orientation of new employees to the nursing unit and the clinical in-service of non-professional staff.
Working Conditions

Working conditions may involve exposure to infectious disease, blood, body fluids, and/or tissue as well as other unpleasant elements such as accidents, injuries, and illness. May be exposed to and be required to handle varying unpredictable, emergency, and crisis situations. Occasionally subjected to irregular hours and pressure due to multiple calls and inquiries.

Requirements
  • A license and current registration to practice as a practical nurse in New York State; OR
  • A limited permit to practice as a practical nurse in New York State; OR
  • An application on file for a limited permit to practice as a practical nurse in New York State.

Note: If you are appointed with either qualification 2 or 3, you must obtain your license within one year of the date your limited permit was issued or steps will be taken to remove you from the position.

Preferred Qualifications
  • One year of healthcare experience working with an adult or aging population in a clinical setting.
  • One year of long-term care experience.
  • Experience using EMR.
