Job Description:

MEDSTAR HEALTH is seeking an accomplished Community Outreach Navigator to assist patients in achieving recovery from high-risk substance use problems.

Primary Responsibilities:

  • Establish recovery plans for patients and connect them with community support services and substance treatment services.
  • Coordinate appointments and transportation for patients as needed.
  • Collaborate with nursing staff and social work team members to ensure comprehensive patient care.
  • Identify patient needs for community support services and provide referrals accordingly.
  • Develop a supportive environment by promoting coping techniques and self-help strategies.
  • Engage in ongoing training to enhance skills and knowledge.
  • Provide family members with recovery resources and support materials.
  • Regularly offer telephone outreach to patients to facilitate continued peer support.
  • Follow up closely on missed treatment appointments and verify patient attendance.
  • Collaborate with Emergency Department staff and social workers to identify community resources.

Requirements:

The ideal candidate will have a strong understanding of recovery principles and community resources, with excellent communication and interpersonal skills. A minimum salary of $66,000 per annum is expected for this role, depending on experience.
