Key Responsibilities- Assessment and Care Planning: Conduct comprehensive assessments of patients and their families to identify their unique needs and develop individualized care plans.
- Collaboration and Communication: Work closely with the interdisciplinary team, including physicians, social workers, and spiritual care advisors, to ensure seamless care coordination and effective communication.
- Patient and Family Support: Provide emotional support, education, and guidance to patients and their families, addressing their physical, emotional, and spiritual needs.
- End-of-Life Care: Deliver high-quality end-of-life care, including symptom management and comfort care, in accordance with NorthStar Care Community indicators.
- Documentation and Reporting: Maintain accurate and timely documentation of patient care, including progress notes, care plans, and other relevant information.
- Quality Improvement: Participate in quality assurance processes and contribute to the development of care delivery goals, strategic planning, and process improvement activities.
- Education and Licensure: Graduation from an accredited school of nursing with a current license to practice as a registered professional nurse in the State of Michigan.
- Experience: Minimum of one to two years of varied nursing experience, with hospice experience and CHPN certification preferred.
- Skills and Abilities: Excellent communication and interpersonal skills, sound judgment, effective organizational and prioritization skills, attention to detail, and the ability to maintain confidentiality and promote positive relationships.
