Job Overview

About the Role

The Lead LPN will provide direct and individualized nursing care to assigned patients under the direction of a registered nurse or provider. You will observe, report, and record medical information and nursing care provided in the electronic medical record. Additionally, you will instruct patients and families according to teaching plans and communicate with the healthcare team to facilitate coordination of individualized patient care.

Responsibilities
  • Provide high-quality, patient-centered care that meets the physical, emotional, and spiritual needs of our patients.
  • Collaborate with interdisciplinary teams to develop and implement comprehensive care plans that address the unique needs of each patient.
  • Conduct thorough assessments of patients' conditions and identify opportunities for improvement.
  • Develop and implement educational programs for patients and families to promote health and wellness.
Requirements
  • CURRENTLY LICENSED TO PRACTICE AS A LICENSED PRACTICAL NURSE BY THE STATE OF EMPLOYMENT OR HOLDS A MULTISTATE LPN LICENSE THROUGH THE INTERSTATE NURSE LICENSURE COMPACT.
  • BASIC LIFE SUPPORT (BLS) REQUIRED PRIOR TO COMPLETION OF ORIENTATION.
