In this challenging role as a Home Health Physical Therapist Specialist, you will have the opportunity to work with patients in their homes, providing one-on-one therapy sessions to help them recover from injuries or illnesses.
About the JobWe are seeking an experienced and compassionate Physical Therapist to join our team at LHC Group. As a Home Health Physical Therapist, you will be responsible for assessing and evaluating patient care needs, developing treatment plans, and performing interventions to improve patient outcomes.
- Evaluate patient's functional status and physical therapy needs
- Assist physician in developing therapy plan of care
- Treat patients to relieve pain, develop and restore function
- Instruct and inform healthcare team, patient, and family/caregivers regarding plan of care
To be successful in this role, you will need to have a strong background in physical therapy, including licensure and CPR certification. You must also have a current driver's license and access to reliable transportation.
