Position Details:

Status: Non-Exempt, Full-time

Work Schedule: Monday to Friday, 8 am - 5 pm; or Thursday to Monday, 8 am - 5 pm (hours may vary on weekends)

Compensation and Benefits:

  • Starting Salary: $66,560 Annually, depending on experience

Key Responsibilities:

Direct Support:

  • Facilitate a supportive environment for clients to:
    • Establish personal program objectives.
    • Identify actionable steps to reach these objectives.
    • Determine necessary resources for successful learning and emotional well-being.
  • Collaborate with clients to formulate measurable goals and objectives for inclusion in Individual Support Plans (ISP).
  • Organize annual meetings with clients and their support network to review and update ISPs.
  • Conduct regular assessments with clients to monitor progress and adapt strategies as needed.
  • Provide support during breaks and teach essential social skills for conflict resolution and emotional management.
  • Encourage self-advocacy and emotional regulation skills to promote client independence.
  • Work with staff to ensure clients are informed about schedule adjustments and assist in course selection aligned with their goals.

Case Management:

  • Document and develop effective strategies to track client progress.
  • Collaborate with departmental leaders to create and implement learning plans that align with our mission.
  • Act as a representative for Meristem in relevant meetings with stakeholders.
  • Maintain open communication with client families and other involved parties.
  • Ensure compliance with program standards and regulatory requirements.
  • Provide updates on client progress and needs to staff and stakeholders.
  • Facilitate communication during critical transitions for clients.
  • Coordinate with the Work Skills team regarding internship and job placement opportunities.

Training and Supervision:

  • Oversee the execution of Individual Support Plans across departments through direct engagement with clients and staff.
  • Mentor new staff and participate in the hiring process.
  • Support staff in their roles and provide guidance on client support.
  • Review and coach staff on documentation practices to ensure compliance with reporting standards.
  • Regularly meet with instructional staff to share insights and address any concerns.

Documentation and Reporting:

  • Prepare progress reports and monthly updates for each client based on observational data and team input.
  • Compile client profiles for instructor review during the enrollment process.
  • Present findings from observations and make recommendations for client enrollment.
  • Draft Intake Assessments and ISPs that reflect client goals and progress.

Additional Responsibilities:

  • Participate in staff meetings and ongoing training.
  • Respond appropriately to emergencies.
  • Adhere to all company policies and procedures.
  • Perform other assigned duties as necessary.

Qualifications:

  • Minimum one year of experience working with neurodiverse individuals or in a related field.
  • Educational background and knowledge of developmental disabilities.
  • Bachelor's degree or equivalent experience in a relevant field.
  • CPR, First Aid, and ProAct certification required or willingness to obtain upon hire.

Preferred Experience:

  • Familiarity with Regional Centers, Department of Rehabilitation, or Department of Developmental Services.

Background check and drug screening required prior to employment.
