Job Summary:

As a key member of our care team, the Licensed Practical Nurse - Care Coordinator will be responsible for ensuring the highest level of quality and continuity of resident care. This role will involve supervising the performance of all members of the nursing care team on a designated neighborhood and shift.

Key Responsibilities:
  • Supervise and coordinate the delivery of resident care, ensuring that all team members are working together to meet the needs of our residents.
  • Maintain accurate and up-to-date resident care records, including medication administration and treatment plans.
  • Communicate effectively with residents, families, and healthcare teams to ensure seamless care transitions.
  • Collaborate with the Clinical Manager and RN nursing supervisors to develop and implement resident care plans.
  • Provide guidance and support to nursing staff, including training and orientation.
  • Ensure a safe and pleasant environment for residents and staff, promoting a culture of respect and compassion.
  • Participate in quality improvement initiatives, identifying areas for improvement and implementing changes to enhance resident care.
  • Assume accountability for compliance with federal, state, and local regulations, as well as Messiah Lifeways' policies and procedures.
Requirements:
  • LPN licensure in Pennsylvania
  • Certification in CPR by an approved program
  • One year of clinical experience preferred
  • Strong communication and leadership skills
  • Ability to work collaboratively as part of a team
  • Commitment to providing high-quality resident care
