Job Summary

We are seeking a skilled Nurse Care Coordinator to join our team at Dresher Hill Health & Rehabilitation. As a key member of our healthcare team, you will be responsible for completing the Resident Assessment Instrument (RAI) process in accordance with state and federal laws, as well as our community's policies and goals.

Responsibilities
  • Complete the RAI process in a timely and accurate manner
  • Collaborate with healthcare professionals to ensure comprehensive care plans
  • Develop and implement individualized care plans that meet the unique needs of each resident
  • Conduct regular assessments and updates to ensure the highest level of care
  • Communicate effectively with residents, families, and healthcare teams to ensure seamless care coordination
  • Participate in quality improvement initiatives to enhance the overall quality of care
Requirements
  • Active, unencumbered Registered Nurse license in the state of practice
  • One year of experience in working with the RAI process
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Strong analytical and problem-solving skills
