This role is responsible for providing personal care and assisting in the care of assigned patients under the guidance and supervision of a registered nurse or therapist.
Key Responsibilities- Provide appropriate care to patients of various ages, adhering to the principles of growth and development over the life span.
- Collaborate with healthcare professionals to ensure exceptional patient care, including primary RNs, physical therapists, and on-call nurses.
- Manage weekend rotation assignments, maintaining open communication with on-call nurses regarding patient conditions and special incidents.
- Document patient care consistently, adhering to Bridge Home Health and Hospice Guidelines.
- Ensure compliance with home exercise programs and promote patient satisfaction through service excellence.
- Comply with organization policies, procedures, and practices to maintain quality job performance and Conditions of Participation from Medicare and Medicaid.
- Current STNA registry in the State of Ohio is required.
- One year of experience as an STNA is preferred.
- A valid driver's license is required, and the individual must meet BVHS's company fleet policy requirements.
- Positive service-oriented interpersonal and communication skills are essential.
- High school diploma is preferred.
This role requires a full range of body motions, including intermittent walking, lifting, bending, squatting, kneeling, twisting, and standing. The associate will be required to walk for up to one hour a day, sit for four to six hours a day, and stand for one hour a day. The individual must be able to lift twenty to fifty pounds and reach work above the shoulders.
