Home Health LPN Job Description

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our Home Health team. As an LPN, you will play a vital role in providing high-quality patient care in the comfort of their own homes.

Responsibilities:
  • Complete evaluations, assessments, and treatments in accordance with policy and the individualized care plan
  • Complete all documentation and records in a timely and accurate manner
  • Follow established policies and procedures as related to the scope of practice
  • Maintain continuity of patient care in a home care setting
