About the Role

We are seeking a compassionate and skilled Family Nurse Practitioner to join our team at Next Care Hospice. As a member of our interdisciplinary team, you will provide high-quality patient care and support to individuals and their families during the end-of-life journey.

Responsibilities
  • Assess, diagnose, and develop referral orders for assigned patients
  • Conduct in-home visits and rounding at local hospitals
  • Collaborate with the attending physician, Hospice Medical Director, and other team members to ensure comprehensive care
  • Utilize EMR software to record patient notes and maintain accurate records
Requirements
  • Current NP license within the state
  • Experience in hospice or palliative care (or willingness to learn at an accelerated pace)
  • Knowledge of EMR software
  • Ability to work cooperatively with the team and maintain a calm demeanor in high-pressure situations
