CenterWell Senior Primary Care is seeking a skilled Nurse Practitioner to join our team in Glendale, AZ. As a Nurse Practitioner, you will play a vital role in providing high-quality patient care and contributing to the success of our organization.
Job Summary:The Nurse Practitioner will work in a team-based care environment, collaborating with physicians, advanced practice professionals, and other healthcare providers to deliver exceptional patient care. You will evaluate and treat patients, follow established standards of care, and monitor patient outcomes using available data and chart reviews.
Responsibilities:- Evaluate and treat patients in accordance with standards of care
- Follow established standards of care and monitor patient outcomes
- Assist in the coordination of patient services, including specialty referrals, hospital and SNF coordination, durable medical equipment, and home health care
- Act as an active participant and lead with the care team through daily huddles
- Help Regional Medical Director, Physician, and Center Administrator in setting a tone of cooperation in practice by displaying a professional and approachable demeanor
- Complete all medical record documentation in a timely manner, working with a quality-based coder to optimize coding specificity
- Follow policy and protocol defined by Clinical Leadership
- Meet with RMD about quality of care, review of outcome data, policy, procedure, and records issues
- Participate in potential growth opportunities for new or existing services within the Center
- Participate in the local primary care "on-call" program of CenterWell Senior Primary Care
- Assure personal compliance with licensing, certification, and accrediting bodies
- Bachelor's degree
- 8 or more years of technical experience
- 2 or more years of project leadership experience
- Active, unrestricted Nurse Practitioner/PA license in the appropriate state
- This role is considered patient-facing and is part of Humana/Senior Bridge's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.
- Must be passionate about contributing to an organization focused on continuously improving consumer experiences
- Masters in Science of Nursing/PA
