At Aveanna Healthcare, we are seeking a compassionate and skilled Hospice Admissions Registered Nurse RN to join our team. As a key member of our hospice care team, you will be responsible for providing high-quality patient care and coordinating with our interdisciplinary team to ensure seamless care delivery.

Key Responsibilities:
  • Deliver and coordinate quality patient care in compliance with physician orders.
  • Continuously observe and assess patient condition and care needs, reporting changes to the supervisor and/or physician as necessary.
  • Document all activities, assessments, nursing actions, responses, and coordination of care in a timely manner.
  • Develop, implement, and update the nursing care plan to meet individual patient needs.
  • Take appropriate nursing action based on assessment and achieve expected outcomes.
  • Recognize changes in patient needs and responses requiring intervention and implement care to prevent risk or reduce risk.
  • Accept responsibility for personal and professional accountability by complying with Aveanna policies, state and federal regulations, accrediting bodies, and the Nurse Practice Act.
  • Provide care utilizing infection control measures that protect both staff and patient according to OSHA standards.
  • Educate patients and families regarding disease processes, self-care techniques, and prevention strategies to meet patient nursing needs.
