About the Role

We are seeking a dedicated and detail-oriented Patient Retention Coordinator to join our team at JWCH Institute, Inc.

This is a part-time position requiring a minimum of 30 hours per week. The estimated annual salary for this role is $62,400-$75,600, depending on experience and qualifications.

The selected candidate will work closely with our Data 2 Care Team to implement strategies for patient identification and engagement. They will serve as the primary service delivery staff for this program, relinking and retaining patients in HIV medical care and connecting them to supportive services.

Main Responsibilities:

  1. Implement patient outreach, identification, and engagement strategies.
  2. Conduct outreach including in-person home visits, shelter visits, and last known locations.
  3. Establish and maintain regular communication with patients to build rapport and support their needs.
  4. Coordinate referrals and linkages to HIV medical care, MCC, and other support services.
  5. Participate as a member of the health care team to gather information on patients' home environments, family relationships, and other factors impacting illness, treatment, and recovery.

Key Skills:

  • Bachelor's degree in Social Work, Psychology, or an equivalent social science degree.
  • At least one year of experience in caring for LGBTQIA+ PLWH.
  • Excellent communication and interpersonal skills.
  • Ability to maintain confidentiality and adhere to ethical standards in patient care.
