About the Role

Key Responsibilities
  • Provide medication management through comprehensive evaluations of patients' medication regimens to ensure safe and effective utilization.
  • Collaborate with patients, providers, nurses, and other healthcare professionals to ensure optimal patient outcomes.
  • Participate in clinical meetings and training sessions to maintain proficiency in pharmacy skills and theory.
  • Develop and implement patient care plans that address medication-related issues and promote optimal patient outcomes.
  • Document patient information and medication therapy plans in a timely and accurate manner.
Requirements
  • PharmD degree from an accredited college of pharmacy.
  • At least 2 years of experience in managing patients' specialty medication needs or clinical pharmacist hospital practice experience.
  • Excellent verbal and written communication skills.
  • Proficiency in Microsoft Office.
  • Current State pharmacy license.
  • Preferred certifications: CSP, BCOP, BCPS or similar.
