We are seeking a highly skilled and compassionate Licensed Vocational Nurse (LVN) to join our team as a Case Manager at Community Services Agency. As an LVN Case Manager, you will play a vital role in providing exceptional care to our clients, ensuring their health and well-being are met.
Responsibilities:
- Provide case management services to frail elderly clients with chronic health conditions, including coordination of post-hospital discharge appointments with primary care physicians.
- Monitor blood pressure and provide chronic disease management educational materials to clients.
- Coordinate support services and link clients to community resources to help them better manage their chronic health conditions and avoid hospitalization/readmission to the hospital.
- Collaborate with healthcare teams to develop and implement individualized care plans.
- Conduct regular assessments and evaluations to ensure clients' needs are met.
Requirements:
- Current California License as a Licensed Vocational Nurse.
- Graduate of an approved LVN School of Nursing.
- Minimum of two years of nursing experience working with older adults.
- Ability to handle multiple projects, meet deadlines, manage priorities, and workflow while maintaining a positive, patient, and calm demeanor.
- Excellent interpersonal and listening skills.
- Ability to exercise excellent judgment, take initiative, and make thoughtful and timely decisions.
- CPR certification.
