Job Summary

The Salvation Army is seeking a skilled Case Manager to provide comprehensive support and expertise to clients in need. As a key member of our team, you will be responsible for assessing client needs, developing and implementing individual service plans, and ensuring continuity of care and cost effectiveness.

Key Responsibilities
  • Conduct intake assessments with new clients
  • Develop and implement individual service plans to meet client needs
  • Provide ongoing support and expertise to clients
  • Collaborate with other agencies and organizations to ensure seamless service delivery
  • Maintain accurate records and reports
Requirements
  • Bachelor's degree in Social Work or related field
  • Minimum 1 year experience working with low-income and/or homeless individuals and families
  • Valid California driver's license
  • CPR/First Aid certification
Preferred Qualifications
  • Bilingual, English/Spanish
  • Knowledge of mental health disorders
  • Proficient in Microsoft Windows and use of Word and Excel
Working Conditions

The Case Manager will work in a fast-paced environment, requiring strong communication and organizational skills. Ability to walk, stand, bend, squat, climb, kneel, and twist on an intermittent or sometimes continuous basis. Ability to lift up to 25lbs. Ability to operate computer, fax, and telephone.
