Key Responsibilities- Identify and Prioritize Patients in Need of Care Management Services
You will utilize a holistic approach to identify patients who require care management services, taking into account biopsychosocial, functional, cultural, spiritual, and financial factors. You will work closely with patients, families, caregivers, and other members of the healthcare team to develop and implement personalized care plans that maximize healthcare responses, quality, and cost-effective outcomes.
Maintain Clear and Concise DocumentationYou will maintain accurate and timely documentation in the patient record, reflecting physical and functional limitations, psychosocial characteristics, educational needs, financial, and transitional needs. You will also initiate referrals to disciplines as indicated and ensure that patient and caregiver treatment goals and preferences are incorporated into the transition of care planning.
Support and Promote Assertive, Proactive CareYou will work collaboratively with revenue cycle partners to remove barriers and ensure patients are converted to the appropriate classification as guided by the physician. You will also participate in nursing unit and department clinical outcome projects, as well as process improvement initiatives within care management.
Support Compliance and Regulatory RequirementsYou will support and follow compliance rules and regulations, including CC 44, IMM, ABNS, HINs, Moon, Bundle letters, and Conditions of Participation for DC Planning and Utilization Management. You will also identify potential or current patient situations that require referral to other members of the healthcare team.
Requirements- Licensure and Certification
You must have a current RN license in the state in which you are working or covered by compact. A BLS Basic Life Support certification from the American Heart Association is also required. A Case Management Certification (ACM, CCM, or ANCC) is preferred.
Education and ExperienceYou must have an Associate degree in Nursing and at least 1 year of experience in a clinical setting. Experience in an acute care clinical setting and ambulatory or post-acute care coordination is preferred.
