Professional Case Management is seeking a skilled Nurse Care Specialist to provide high-quality patient care in the comfort of clients' homes. As a key member of our team, you will be responsible for assessing and evaluating clients' health status, developing and implementing personalized care plans, and providing education and support to clients and their families.
Key Responsibilities:
- Assess and evaluate clients' health status, identifying areas of need and developing comprehensive care plans
- Provide direct patient care, including administering medications, wound care, and other treatments as prescribed
- Collaborate with clients, families, and healthcare providers to ensure seamless care coordination
- Document client progress, identifying areas of improvement and making recommendations for future care
- Participate in ongoing education and training to stay current on best practices and industry developments
Requirements:
- Graduate of a state-approved school of professional nursing (BSN preferred)
- Current, unrestricted license as an RN in the state(s) of practice
- Current CPR certification and TB test
- Strong communication and interpersonal skills, with ability to work effectively with diverse populations
