Job Summary

The Healthy Families America Program Assistant plays a vital role in providing comprehensive support and resource coordination to families participating in the HFA program. This position involves developing family support plans, linking families to community resources, promoting positive parent-child interactions, and managing administrative duties.

Key Responsibilities
  • Family Support
    • Assessment: Initiate contact with referrals, track progress, and complete monthly reports on referral rates, enrollment percentages, and outcomes.
    • Initial Interviews: Schedule and conduct eligibility assessments and parent surveys.
    • Resource Coordination: Connect families with community resources like healthcare, housing, education, and childcare services.
    • Advocacy: Ensure families receive appropriate services by advocating within the community and with other providers.
    • Cultural Sensitivity: Provide services in a culturally sensitive manner, respecting family diversity.
    • Establish Trust: Build trusting relationships with at-risk families by maintaining consistent contact.
    • Client Confidentiality: Maintain confidentiality in all interactions and documentation.
  • Administrative and Program Support
    • Health Tracking: Monitor well-baby visits and immunizations for enrolled families.
    • FROG Scale Assessment: Complete Family Resilience and Opportunities for Growth (FROG) Scale assessments.
    • Data Management: Ensure accurate documentation of assessments and services, enforce data policies, and implement secure data handling procedures.
    • Collaboration: Work with team members and supervisors for cohesive service delivery, participating in regular meetings and trainings.
    • Data Analysis: Analyze and interpret assessment and immunization data, identify patterns and problems, and generate detailed reports for management.
    • Community Promotion: Promote the Healthy Families program to encourage referrals.
    • Material and Supply Management: Maintain inventory of program materials and supplies.
Qualifications/Education
  • Education: Bachelor's degree in social work, psychology, child development, human services, data management, or related fields. Equivalent experience may be considered.
  • Experience: Minimum of 2 years in family support, home visiting, or early childhood services. Experience with the Healthy Families America model and at least 2 years in data management and policy enforcement preferred.
Skills
  • Strong interpersonal and communication skills.
  • Ability to build trust and establish rapport with families.
  • Knowledge of community resources and navigation of service systems.
  • Proficient in documentation, case management, and data analysis.
  • Ability to work independently and as part of a team.
  • Cultural competence and sensitivity to diverse populations.
  • Spanish-speaking ability is a plus.
