Job Summary

Central City Concern is seeking a highly skilled and compassionate Street Outreach Coordinator to join our team. As a key member of our LEAD program, you will be responsible for providing screening, outreach, and service brokerage to participants referred to the program.

You will work closely with a multi-disciplinary team to coordinate with community resources, broker services, and provide access to address client needs. This includes substance abuse treatment, medication management, physical health treatment, mental health treatment, benefits and entitlements, appropriate and affordable housing, employment and educational opportunities, and supports for basic needs.

The ideal candidate will have a strong understanding of community and social service agencies, including health services, housing, employment, benefits, and other social services. You will also possess excellent communication and interpersonal skills, with the ability to work effectively with staff, clients, and outside contacts from diverse backgrounds.

Key Responsibilities
  • Provide street outreach to engage individuals who have been referred to LEAD with the purpose of facilitating completion of the intake process.
  • Develop and maintain positive, collaborative relationships with all LEAD partners, including law enforcement, human services, and district attorney's office.
  • Complete initial screening and intake with all new referrals, assessing and supporting accessing services for emergent/immediate client needs.
  • Provide street outreach and short-term case management services to engage individuals who have been referred to LEAD with the purpose of facilitating completion of the intake process.
  • Manage, coordinate, and facilitate the assignment/hand off of new referrals to a case manager, peer, or housing.
Requirements
  • HSD/GED required.
  • Education and experience appropriate for this role, as demonstrated by one of the following: QMHA-R, CADC-R, Bachelor's Degree in a service-related field.
  • Experience working with homeless individuals and providing street outreach, or outreach-based community social service or healthcare environment preferred.
  • Lived experience preferred.
  • Bi-lingual preferred.
  • Must have current CPR certification prior to start.
