Clinical Care Coordinator

We are seeking a highly skilled Clinical Care Coordinator to join our team at Excelin Health Services. This role plays a vital part in ensuring the delivery of high-quality care to our patients.
About the RoleThe Clinical Care Coordinator will provide administrative support to our clinical managers and care staff, coordinating daily reports, patient supplies, and interagency referrals. They will also work closely with various departments, facilities, and physicians to coordinate appointments and obtain necessary documents for care delivery and billing.
This position requires a licensed clinician with the ability to receive physician orders. The ideal candidate will possess excellent organizational skills, be able to prioritize tasks effectively, and have strong communication skills.
Responsibilities- Schedule Management: Collaborate with the scheduling department to ensure accurate employee and patient schedules, reconcile schedule variances, and verify productivity.
- Patient Support: Communicate with field staff to prioritize patient assignments based on clinical changes or conditions, as directed by the Clinical Manager.
- Care Delivery: Assist with order review to ensure compliance with State and Federal Regulations, supporting safe and effective care delivery.
- Operations and Compliance: Maintain agency Clinical Tasks Reports, identifying and resolving variances such as Schedule Report, Missed Visit Report, Past Due Visits, Schedule Deviation, Visit Exception, and Orphan Documents.
- Clinical Care and Billing: Educate and obtain clinical documents needed for home health billing, supporting operations and compliance.
- Admission Criteria: Verify daily admission criteria necessary for Notice of Admission and communicate variances to the Clinical Manager for follow-up.
- Care Coordination: Assist with coordination of DME, medical supplies, appointments, and referrals to outside agencies.
- Facilitation: Coordinate patient care coordination documents with acute and post-acute facilities during patient transfers.
- Education and Training: Assist with education and training of field staff.
- Coverage: May cover for field staff in situations of high census or other needs.
- Experience: One year of experience in home care or a facility setting.
- Licensure: Must be a licensed clinician with a current, active license in good standing in the state where Excelin Health Services operates.
- Communication Skills: Skilled in making effective verbal and written communications with subordinates, co-workers, consumers, referral agencies, and comfortable with conflict resolution skills.
- Education: Ability to educate consumers on all service lines and present to seniors and aides in a group setting.
- Energy Level: High energy level and passion for care delivery.
- Organizational Skills: Excellent organizational skills and ability to complete competing priorities.
- Knowledge: Thorough understanding of home health or hospice qualifying criteria and coverage guidelines.
- Listening Skills: Ability to listen attentively and offer care options based on individual patient health needs.
- Computer Skills: Proficient computer skills, including Microsoft email, word, excel, power point, and home health or hospice agency software system.
- Vehicle Requirements: Must have a vehicle, current driver's license, and appropriate automobile insurance.
- Office environment with prolonged sitting and keyboard work.
- Some driving/travel involved to conduct patient visits.
- Briefly travel to visit patients when required.
- Must be able to lift 25 lbs. and handle office supplies and equipment.
- Considerable reaching, stooping, bending, kneeling, or crouching.
- Visual acuity and hearing needed to perform required job functions.
The estimated salary range for this position is $62,000 - $72,000 per year, depending on experience and qualifications. We offer a comprehensive benefits package, including medical, dental, vision, and retirement plans, as well as opportunities for professional growth and development.
