Role Summary:
The Outpatient Rehabilitation Specialist delivers Physical Therapy services as prescribed by a physician, adhering to hospital regulations, the State of Connecticut Practice Act, and established quality standards. This role includes overseeing the clinical activities of Physical Therapy Assistants.
Key Responsibilities:
- Formulate individualized care plans in collaboration with patients, addressing their safety, educational, psychological, and developmental needs.
- Analyze and interpret patient data to set and adjust treatment goals, effectively communicating these objectives and methodologies to patients and their families.
- Exhibit proficiency in recommending various therapeutic techniques and modalities.
- Adhere to professional and departmental documentation protocols.
- Implement a home exercise regimen tailored to each patient.
- Maintain a manageable caseload while meeting departmental productivity benchmarks.
- Participate in continuing education as mandated by licensure and conduct educational sessions for rehabilitation staff and other healthcare professionals.
- Engage in regular communication with the Rehabilitation Supervisor, team members, and referring physicians regarding patient care.
- Perform additional job-related tasks as required.
Essential Qualifications:
- Graduation from an accredited Physical Therapy program.
- Current licensure or eligibility for licensure in the State of Connecticut.
Preferred Qualifications:
- Familiarity with electronic medical record systems.
- 3-5 years of clinical experience.
- Adaptability in scheduling.
