The Safe Haven initiative is a housing-first model designed to assist veterans facing multiple challenges, including mental health issues, substance use disorders, and chronic medical conditions, in securing permanent housing. The Case Manager will work closely with veterans to provide comprehensive support services in both individual and group settings.
Key responsibilities include:
- Assisting veterans in finding permanent housing through tailored support services.
- Employing a client-centered and trauma-informed approach to help veterans develop essential skills for maintaining housing.
- Collaborating with veterans to assess needs, create goal plans, and ensure follow-up.
- Facilitating access to housing options, community resources, and necessary documentation.
- Providing transportation and tracking appointments related to housing and healthcare.
The Case Manager will also lead weekly group sessions covering vital topics such as life skills, effective communication, and coping strategies. Case management services will be delivered in partnership with other program staff, clinical supervisors, and external collaborators.
Timely documentation is essential, including case notes, goal plans, and veteran activities, utilizing an electronic record system. Adherence to the standards set by Volunteers of America, the Veterans Administration, and the Commission on Accreditation of Rehabilitation Facilities (CARF) is mandatory.
