Job Summary

In this expanded healthcare role, you will act as a liaison between patients and their families, primary care providers, specialists, and other members of the healthcare team. You will ensure the delivery of quality, efficient, and cost-effective healthcare services for those with complex conditions and complex social needs.

Responsibilities
  • Act as a liaison between patients and their families, primary care providers, specialists, and other members of the healthcare team.
  • Ensure the delivery of quality, efficient, and cost-effective healthcare services for those with complex conditions and complex social needs.
  • Develop and implement care plans that meet the unique needs of each patient.
  • Collaborate with healthcare teams to coordinate patient care and services.
  • Provide education and support to patients and their families to promote healthy behaviors and self-management of chronic conditions.
Requirements
  • Licensed as a Registered Nurse in the State of Michigan.
  • Bachelor of Science degree in nursing preferred.
  • Five years' experience as an RN preferred.
  • High-level understanding of complex chronic disease management.
  • Self-directed professional with the ability to make independent judgments directing patient care.
