Our HouseCalls program provides annual clinical assessments to patients in the comfort of their own homes. This vital work helps identify and reduce health risks, while also coordinating follow-up care to improve overall well-being.

Key Responsibilities:

  • Conduct in-home assessments, including medical history reviews, medication reconciliation, and comprehensive physical exams
  • Perform evidence-based practice screenings, including point-of-care testing as needed
  • Identify diagnoses and develop care management plans to inform member's primary care physician
  • Educate patients on disease processes, medication, and compliance
  • Intervene in urgent and emergent situations as needed

Requirements:

  • Active NP or PA license, with ability to obtain national certification in state of assignment
  • Ability to complete physical requirements of the job, including lifting and navigating various dwelling conditions
  • Access to reliable transportation for travel up to 85% within licensed states

Preferred Qualifications:

  • 1+ years of clinical experience in highest level of education, clinical setting
  • Experience in Gerontology, Cardiology, Internal Medicine, or Endocrinology
  • Home Health care or home visit experience

Washington Residents Only: The salary range for this role is $88,000 to $173,200 annually, with comprehensive benefits and incentives.
