We are seeking a highly skilled Licensed Clinical Social Worker to join our WISE program, providing mental health services to seniors, individuals with medical conditions, or those with intellectual disabilities, and their families/caregivers. This part-time Fee for Service role offers flexibility to work in clients' homes on your own schedule, delivering care that significantly enhances their quality of life while maintaining the highest standards of ethics and confidentiality.

This is a clinical role that provides in-home, non-crisis outpatient mental health counseling. LCSWs have autonomy to manage their own schedules, allowing them to set appointment times and arrange their work hours based on client needs and personal preferences. This includes the ability to work evenings or weekends if desired, though not required.

Responsibilities:
  • Assess the mental and behavioral health status of referred clients with mild to moderate mental health conditions and/or substance abuse issues
  • Conduct standardized counseling/talk therapy sessions for Stokes Counseling clients
  • Perform intake/biopsychosocial assessments to evaluate clients' mental and behavioral health status
  • Formulate differential diagnoses along with short-term and long-term goals
  • Provide and maintain written documentation of intake, assessments, progress/contact notes, interventions, treatment plans, and termination notes within the designated time frame
Requirements:
  • Licensed Clinical Social Worker
  • 1 - 3 years experience conducting counseling/talk therapy sessions with diverse client populations
  • Previous experience working in the medical field with seniors and/or intellectually disabled clients
  • Passion for in-home work
  • Ability to work collaboratively in a team-oriented environment
  • Advanced proficiency in conducting intake assessments and formulating treatment plans
  • Strong written communication and interpersonal skills
  • Have a valid driver's license and access to a properly insured vehicle for work or reliable transportation
