Job Summary

We are seeking a highly motivated and experienced Qualified Mental Health Professional (QMHP) Case Manager to join our team at Carolina Therapeutic Services. As a QMHP Case Manager, you will be responsible for providing case management services to individuals with mental health needs, including coordinating behavioral health services, engaging community and natural supports, and delivering direct care services when necessary.

Key Responsibilities:

  • Coordinate behavioral health services and other interventions for consumers with other professionals and Child and Family Team members.
  • Engage community and natural supports, and include their input in the person-centered planning process.
  • Deliver direct care services when necessary to ensure consumer continuity of care.
  • Complete the ongoing revision of the Individual Treatment Plan and ensure its implementation.
  • Monitor and document the status of the recipient's progress and the effectiveness of the strategies and interventions outlined in the Treatment Plan.
  • Collect data and submit for continuing service authorization, maintaining up-to-date service authorization tracker to sustain 100% authorizations on all consumers within team.
  • Input and upload consumer demographics onto documentation site (OnTarget).
  • Consult with identified medical and non-medical providers.
  • Work with a team approach with other employees to ensure agency compliance.
  • Report critical concerns and problems to Supervisor in a timely manner and actively seek Supervisor's input and assistance to resolving the concern.
  • Actively participate in an appropriate manner with the decision-making process as a pro-active team member and cooperate with team's decision, whether programmatic or clinical.
  • Facilitate flow of information.
  • Ensure Client Rights are being adhered to for all consumers.
  • Attend weekly and monthly meetings with State Directors, Clinical Director, and supervisor.
  • Report to supervisor on a weekly basis on progress or regress of team case load.

Supervisory Responsibilities:

  • Monitor service delivery to ensure goals are achieved to meet consumer needs and ensure continuity of care.
  • Conduct weekly service follow-up/verification calls to consumers/guardian.
  • Conducting monthly outcome summaries and progress of consumer goals. Complete discharge summaries within 7-days of discharge.
  • Responsible to ensure that services are provided in a manner, which meets all applicable regulatory licensure and accrediting regulation.
  • Provide weekly supervisions to assigned staff, must be documented.
  • Review/Approve all assigned staff's service notes to ensure they are billable and in accordance with regulatory standards.
  • Create/Review and assign service schedules for assigned staff.
  • Provide routine monitoring for services being rendered by team members for quality assurance in accordance with Medicaid Part 132 requirements.
