Job Summary

We are seeking a highly skilled Physical Therapist Assistant to join our Home Health team in the Central Territory of St. Louis. As a Physical Therapist Assistant, you will work closely with our Physical Therapist team to provide high-quality patient care and support the rehabilitation process for patients in their homes.

Responsibilities
  • Assist the Physical Therapist in data collection, including tests and measurements, and communicate status updates within the scope of practice.
  • Implement the therapy plan of care for assigned patients, including performance of therapy procedures and related tasks delegated by the supervising Physical Therapist.
  • Progress treatment techniques and procedures as patient status and functional potential change, in conjunction with the specific therapy plan of care designed by the supervising Physical Therapist.
  • Identify, respond, and report to the supervising Physical Therapist any changes in the patient's physical/mental status and/or tolerance to treatment.
  • Under the direction of the supervising Physical Therapist, coordinate treatment programs with other clinical/therapeutic services and available community resources, attending appropriate clinical meetings to facilitate coordination.
  • As delegated by the supervising Physical Therapist, provide patient education, including development of comprehensive and progressive home programs with instruction to the patient, family, and other caregivers as appropriate to facilitate a safe and effective discharge.
  • As delegated by the supervising Physical Therapist, prepare for patient discharge, including ordering or fabrication and adaptation of necessary equipment, as well as recommendations for therapeutic services ensuring the continuum of care.
  • Assist with direction of functional training classes and/or programs under the supervision of the licensed Physical Therapist as assigned.
  • Assign routine tasks to aides and clerical staff related to their job function in conjunction with the supervising Physical Therapist.
  • Demonstrate flexibility in all aspects of caseload management, including accepting coverage, and effectively and efficiently manage individual, concurrent, and group treatment sessions based on patient care needs.
  • Complete medical record documentation in a clear, concise, accurate, and timely manner, inclusive of daily documentation and charge entry using approved formats in compliance with state, federal, and intermediary guidelines and in accordance with the facilities' policies and procedures.
  • Under the direction of the supervising Physical Therapist, participate in the therapy assistant student program as assigned and appropriate, providing a positive learning environment for therapy assistant program students with appropriate supervision.
  • Actively create an individualized professional development plan with clinical management, identifying and initiating own professional growth, actively pursuing professional development through continuing education, self-study, and involvement in professional activities and organizations.
  • Actively participate in patient and professional advocacy efforts, including lobbying activities, health fairs, community-level events, professional memberships, fundraising/charitable activities.
  • Adhere to discipline-specific State Practice Act and other regulatory agencies, maintaining professional licensure in good standing and completing all state-mandated continuing education requirements.
  • Understand the organization's performance improvement methodology to improve the quality of services provided by this position, demonstrating awareness of organizational performance improvement initiatives and following through with action plans.
Qualifications
  • Current state licensure or eligibility for state licensure as a Physical Therapy Assistant required.
  • Certified BLS or completion within the first 90 days of employment required.
  • One or more years of clinical experience.
  • Valid state Driver's License.
