Job Summary

We are seeking a highly skilled Physical Therapist Assistant to join our Home Health team in the Central Territory of St. Louis. As a Physical Therapist Assistant, you will work closely with our Physical Therapists to provide high-quality patient care and support in the home environment.

Responsibilities
  • Assist Physical Therapists in data collection, including tests and measurements, and communicate status updates within their scope of practice.
  • Implement the therapy plan of care for assigned patients, including performance of therapy procedures and related tasks delegated by the supervising Physical Therapist.
  • Progress treatment techniques and procedures as patient status and functional potential change, in conjunction with the specific therapy plan of care designed by the supervising Physical Therapist.
  • Identify, respond, and report to supervising Physical Therapist any changes in patient physical/mental status and/or tolerance to treatment.
  • Coordinate treatment program with other clinical/therapeutic services and available community resources, attending clinical meetings to facilitate coordination.
  • Provide patient education, including development of comprehensive and progressive home programs with instruction to patient, family, and other caregivers as appropriate to facilitate a safe and effective discharge.
  • Prepare for patient discharge, including ordering or fabrication and adaptation of necessary equipment, and recommendations for therapeutic services ensuring the continuum of care.
  • Assign routine tasks to aides and clerical staff related to their job function in conjunction with supervising Physical Therapist.
  • Demonstrate flexibility in all aspects of caseload management, including accepting coverage, and effectively and efficiently manage individual, concurrent, and group treatment sessions based on patient care needs.
  • Complete medical record documentation in a clear, concise, accurate, and timely manner, inclusive of daily documentation and charge entry using approved formats in compliance with state, federal, and intermediary guidelines and in accordance with the facilities policies and procedures.
Qualifications
  • Current state licensure or eligibility for state licensure as a Physical Therapy Assistant required.
  • Certified BLS or completion in first 90 days of employment required.
  • One or more years of clinical experience.
  • Valid state Driver's License.
