Job Summary

Prokatchers is seeking a travel Certified Surgical Technologist for a travel job in a dynamic healthcare setting.

Key Responsibilities
  • Assist in the collaboration, development, implementation, revision, and reporting of the case management program.
  • Act as a liaison between patients, families, nurses, physicians, multidisciplinary teams, and patient healthcare benefactors to optimize outcomes.
  • Provide expert patient care by coordinating a continuum of care for a defined population of patients from pre-admission through post-discharge.
  • Ensure patients and families proceed efficiently through the hospitalization process.
Essential Functions
  • Lead the coordination of patient care with other disciplines within the care team, monitoring the appropriateness and timeliness of care.
  • Ensure the interdisciplinary care plan is consistent with the patient's clinical course, continuing care needs, and covered services by monitoring diagnostic testing, treatments, and procedures.
  • Discuss with physicians the appropriateness of resource utilization, consultations, treatment plans, estimated length of stay, and discharge plans.
  • Focus on complex patients, frequent emergency department utilizers, chronic pain patients, substance abuse patients, and homeless patients.
  • Collaborate with acute care case managers to ensure appropriateness of ongoing care.
  • Coordinate the transfer of patients to tertiary centers, including the transfer of patient information required for continuity of ongoing treatment and services.
  • Provide oversight and collect data required for regulatory and accreditation compliance.
  • Manage frequent emergency department visitors by conducting a focus study review of previous and current admissions.
Requirements
  • Education: Bachelor's Degree in Nursing (BSN) preferred.
  • Experience: Experience in a care management role preferred.
  • Licenses/Certifications: Registered Nurse (RN) licensure in the state of practice required, Case management certification preferred.
