Job Summary

We are seeking a skilled Registered Nurse to join our Home Health team in Sheboygan, WI. As a Registered Nurse - Home Health, you will be responsible for planning, coordinating, and documenting care for an assigned caseload of patients.

Key Responsibilities
  • Develop and implement home health plans that meet each patient's specific needs and are in compliance with accrediting organization standards, federal and state regulations, reimbursement guidelines, and agency policies and procedures.
  • Participate in Quality Assessment and Performance Improvement (QAPI) activities.
  • Collaborate with the Case Manager to develop and implement care plans that meet each patient's specific needs.
Requirements
  • Graduate from an accredited and approved school of nursing accepted for licensure in the state of practice is required.
  • Bachelor's Degree in nursing is preferred.
  • Valid state driver's license, current automobile insurance, and reliable automobile are required.
  • Prior experience in home health or hospice is preferred.
  • 1 year acute nursing experience is preferred.
  • Licensed as a Registered Nurse (RN) in the state of practice is required.
  • Basic Life Support (BLS) is required.
