Social Work Team Lead Inpatient Case Manager

We are seeking a highly skilled and experienced Social Work Team Lead Inpatient Case Manager to join our team at MedStar Franklin Square Hospital.
Key Responsibilities:- Collaborate with the manager to implement, integrate, and operationalize the Case Management Department's direction.
- Facilitate the continuum of care by working collaboratively with the interdisciplinary staff, both internal and external to the organization.
- Provide professional expertise as a vital member of the interdisciplinary team to ensure patients receive the most appropriate and cost-effective care.
- Lead, instruct, supervise, and direct associates in the Case Management Department.
- Supervise, monitor, and document professional progress of social work staff as required by state licensing statuses.
- Participate in ongoing training and professional development opportunities to stay current on best practices and new developments in utilization management.
- Assist with selection, training, orientation, and assignments of department staff.
- Assist with development of performance standards and evaluation of associate performance.
- Assess staff professional development and training needs.
- Participate in staff meetings, Care Management meetings, work groups, and other in-services to enhance professional knowledge and competency in management of patients.
- Collaborate with Primary Care Physicians, Medical Specialists, Home Health, and other ancillary providers to coordinate patient care.
- Provide professional expertise as a vital member of the interdisciplinary team to coordinate transition management for patients and arrange for appropriate care to ensure safe and timely discharge.
- Demonstrate knowledge of resources available in the system and community and utilize them effectively in supporting the staff and patients during care.
- Advise providers and staff on availability of appropriate services for patients who require post-hospital care.
- Identify training needs related to the clinical software system (data entry/documentation) and programs or processes within the Case Management Department.
- Implement a plan for training based on identified needs.
- Provide support to manager's responsibilities and deliverables, including the Case Management Department Plan and Appraisal of LOS, IMM, and MOON delivery.
- Successfully complete 2-3 preceptor and/or staff education assignments per year, following established education plans and guidelines.
- Regularly report progress of new associates to their manager and actively participate in planning and follow-up meetings regarding associate progress.
- Assist with departmental projects and other functions such as support department operations and/or assist with patient-specific issues that may arise.
- Master's degree in social work (LMSW) or master's degree in LGSW (DC hospitals only).
- 1-2 years' Social Work experience preferred.
- 1-2 years' experience in case management preferred.
