Job Summary


The Licensed Practical Nurse (LPN) Clinical Care Supervisor (CCL) in Long-Term Care (LTC) is responsible for employing the nursing process—assessment, diagnosis, outcomes/planning, implementation, and evaluation—to deliver personalized care in a long-term care environment under the guidance of a registered nurse, advanced practice provider, or physician.

This role involves collaboration with inter-professional colleagues to plan, implement, and assess care. Responsibilities include documenting patient/resident history, symptoms, medications, and care provided in accordance with applicable standards and regulations.

The CCL utilizes technology to enhance the quality of patient/resident care, collaborates with team members, effectively delegates tasks, supervises unlicensed personnel, and assumes overall nursing accountability for assigned patients/residents. Additionally, this role may involve serving in leadership capacities on departmental or organizational committees.

Maintaining residents' dignity, privacy, and confidentiality is paramount. The CCL ensures that residents have opportunities for independence and choice in alignment with their care plans.

The role also includes maintaining safe and orderly living environments for residents. The CCL demonstrates competency and practices within the full scope of nursing expertise, adhering to age and population-specific standards as designated for the care area.

Effective communication skills are essential for collaboration, negotiation, and conflict resolution with peers, inter-professional colleagues, patients/residents, and families. Strong clinical reasoning and critical thinking skills are necessary to analyze data and respond appropriately to changing situations. Responsibilities also encompass care coordination, patient/resident assessment, education, triage, and various nursing interventions.


Functions within the scopes and standards of nursing practice as outlined in the Nurse Practice Act and Administrative Rules in the state of practice and licensure.

The Sanford Professional Nursing Practice recognizes the Scope and Standards of Practice and the Code of Ethics for Nurses with Interpretive Statements as published by the American Nurses Association as the foundation of nursing care delivery and professional conduct.

Qualifications

Graduate from an accredited practical nursing program as a Licensed Practical Nurse. An Associate degree in practical nursing is preferred.

If the position requires IV therapy/management skills, the following is required:

  • Graduation from a Board approved AASPN program where IV therapy is part of the curriculum or completion of an IV Therapy course approved by the North Dakota Board of Nursing.
  • Must have attended a pre-licensure nursing program that included supervised clinical experience across the lifespan or have a minimum of 400 hours of experience as an LPN at another facility.

Currently holds an unencumbered Licensed Practical Nurse (LPN) license with the State Nursing board and/or possesses multi-state licensure privileges. Must obtain and maintain required department-specific competencies and certifications, completing annual continuing education units (CEU) and competency training requirements.
