Schedule: Full-Time Nights OR Part-Time Weeknights and Weekends

Pay: $25-$30/hour (LPN) | $35-$45/hour (RN)


Position Overview: Sunflower Communities is seeking experienced Registered Nurses (RN) and Licensed Practical Nurses (LPN) to provide high-quality care to our residents. Successful applicants will have a strong background in geriatric care and be able to work autonomously.

Key Responsibilities:

  1. Develop and implement individualized care plans for residents, ensuring their needs are met and their best interests are prioritized
  2. Conduct thorough clinical assessments of residents' health status and document findings accurately
  3. Manage the administration of medications and monitor the status of chronic illnesses
  4. Collaborate with external healthcare providers and internal team members to ensure seamless care coordination
  5. Communicate effectively with residents' families and loved ones, keeping them informed of changes in their health or care plan

    Requirements:

    1. Licensed as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) in Minnesota
    2. Experience with Electronic Medical Records (EMR) software
    3. Prior nursing experience in a SNF, AL, TCU, or Home Health setting is highly desirable
    4. Demonstrated ability to work independently and as part of a team
    5. Excellent communication and organizational skills
