Job Title: Certified Home Health Aide

Job Summary:

The Certified Home Health Aide is a paraprofessional member of the home health team who works under the supervision of a registered nurse or therapist. The CHHA is responsible for providing various personal care services to meet the patient's needs, including observing patients, reporting observations, and documenting care performed.

Responsibilities:
  • Providing personal care services, including bathing, back rubs, oral hygiene, and changing bed linen
  • Assisting patients with dressing and undressing
  • Providing skin care to prevent breakdown
  • Assisting patients with toileting activities
  • Keeping patient living areas clean and orderly
  • Assisting patients with self-administration of medication
  • Adhering to organization documentation and care procedures
  • Meeting position productivity expectations
Qualifications:
  • Meets training requirements in accordance with State and Federal laws
  • At least 18 years of age
  • Ability to read and follow written instructions and document care given
  • Self-directing with ability to work with little direct supervision
  • Empathy for the needs of the ill, injured, frail, and impaired
  • Possesses and maintains current CPR certification
  • Demonstrates tact, patience, and good personal hygiene
  • Licensed driver with automobile that is insured in accordance with Organization requirements
