Key Responsibilities- Assess and evaluate patients' physical therapy needs, developing personalized plans of care to achieve optimal outcomes.
- Provide physical therapy services, including exercises, modalities, and education, to patients in their homes.
- Collaborate with healthcare teams to ensure seamless care coordination and communication.
- Conduct home safety assessments and provide education to patients and caregivers on safe mobility and fall prevention.
- Document patient progress and outcomes, ensuring accurate and timely reporting to healthcare teams.
- Participate in discharge planning and provide recommendations for continued care and rehabilitation.
- Master's degree in Physical Therapy from an accredited program.
- Current licensure to practice as a Physical Therapist in the state, if applicable.
- At least two years of experience as a Physical Therapist, with a preference for community or home health experience.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Current CPR certification and a valid driver's license with a reliable vehicle.
