Responsibilities
  • Provide Geriatric Care to patients with diverse backgrounds and needs.
  • Work collaboratively with healthcare teams to develop and implement effective care plans.
  • Conduct Assessments and create personalized plans to meet the unique needs of each patient.
  • Develop and maintain strong relationships with patients, families, and healthcare providers.
  • Stay up-to-date with the latest Clinical Guidelines and best practices in geriatric care.
Requirements
  • At least 1 year of experience working with geriatric patients.
  • At least 2 years of experience working with geriatric patients from ethnically diverse groups.
  • Current Licensure as a Clinical Social Worker in the state of Washington.
  • Experience with EPIC electronic health records.
  • Ability to work independently and as part of a team.
  • Strong communication and interpersonal skills.
Preferred Qualifications
  • Experience working in a Home Care setting.
  • Knowledge of Cultural Issues and their impact on patient care.
  • Experience with Discharge Planning and care coordination.
  • Ability to work flexible hours, including evenings and weekends.
