LPN - Women's Health - Casual

Job Summary

We are seeking a skilled Licensed Practical Nurse (LPN) to join our Women's Health team in a casual role. The successful candidate will provide direct patient care, perform lab tests, administer medications, and carry out clinical activities under the direction of the Nurse Manager, RN Supervisor, RN, and/or physician.

Key Responsibilities

1. Deliver direct and indirect patient care in accordance with the LPN Practice Act, hospital, and/or physician practice policies, procedures, protocols, and other professional standards of care.
2. Collect patient health care data, including vital signs, weight, chief complaint, and pain assessment, focusing on the physical, psychosocial, age, cultural, and spiritual needs of the patient.
3. Involve the patient/family and other healthcare providers when appropriate.
4. Complete patient care treatments/testing as ordered by the physician.
5. Administer medications/immunizations using appropriate routes and techniques specific to the patient's age.
6. Comply with the completeness, accuracy, and timeliness of documentation.
7. Provide education and instruction, within their scope of practice, to patients and families.
8. Assist in assessing the patient's educational needs and identifying educational barriers.
9. Provide educational material and/or instructions based on assessed needs.
10. Document all education/instruction provided, including topic, individual provided to, and understanding.
11. Maintain a collaborative relationship with physicians to achieve desired patient outcomes.
12. Prepare patients for examination, treatments, and procedures.
13. Assist the physician with treatments and procedures as requested.
14. Provide for patient safety in compliance with hospital and physician practice policies.
15. Utilize two forms of patient identification before implementing any form of patient care.
16. Label all patient specimens with identifiers per policy.
17. Use patient identifiers on all written documentation, including notes and point-of-care testing results.
18. Assist in compliance with all regulatory agency requirements.
19. Perform quality control testing for all point-of-care testing per policy.
20. Participate in non-punitive medication reporting.
21. Adhere to infection control processes, including standard precautions, to reduce the risk of acquiring or transmitting infections.
22. Demonstrate leadership by utilizing the concepts inherent to the practice of professional nursing.
23. Demonstrate an ability to solve problems independently and seek assistance from the immediate supervisor when appropriate.
24. Maintain accountability for actions taken.
25. Delegate patient care appropriately.
26. Effectively function as a resource person.
27. Act as a patient advocate.
28. Perform effectively in the role of preceptor/mentor to new hires and/or students.
29. Assist in maintaining/decreasing organizational/departmental costs.
30. Assist in the department's compliance with JCAHO and other regulatory agencies.
31. Participate in continuous survey readiness for inspections and surveys.
32. Coordinate communications between patients/families and physician practice, pharmacies, specialists, and insurance carriers.
33. Provide advice based on protocols and physician instructions and direct patients to the appropriate level of care.
34. Facilitate patients' access to prescription refills based on physician's order.
35. Obtain prior authorization and/or assist with indigent programs per physician order based on the patient's financial status or insurance coverage.
36. Support performance improvement initiatives with the physician practice.
37. Assist in data collection as directed by the manager/physician.
38. Incorporate into practice performance improvement initiatives that have been implemented.
39. Maintain the environment of care.
40. Ensure that supplies in patient care areas are replenished daily.
41. Consistently maintain a clean safe environment.
42. Perform point-of-care testing as ordered by the physician.
43. Complete all quality control testing per policy.
44. Identify results that are out of acceptable range and follow up prior to performing patient testing.
45. Use two patient identifiers, name and DOB, prior to testing.
46. Accurately record patient test results on appropriate result forms and testing logs.
47. Prepare forms, copies records, and respond to requests per policies and procedures.
48. Assist in maintaining patients' medical records to facilitate filing and retrieval of information.
49. Participate in all other office functions as necessary.
50. Perform other duties as required.

Requirements

• Graduate of an accredited School of Practical Nursing.
• Strong leadership ability, good organizational skills, independent and critical thinking skills, sound judgment, and knowledge of legal aspects and liability of nursing practice.
• Strong ability to communicate complex and/or controversial topics and concepts to a wide and diverse audience.

Preferred Qualifications

• Previous LPN experience.

License, Certification & Clearances

• Current licensure to practice as a Licensed Practical Nurse in the State of Pennsylvania.
• Current Basic Life Support (BLS) approved curriculum of the American Heart Association (CPR and AED) program.
• Act 33 with renewal.
• Act 34 with renewal.
• Act 73 FBI Clearance.

Supervisory Responsibilities

This position has no direct supervisory responsibilities, but does serve as a coach and mentor for other positions in the department.

Position Type/Expected Hours of Work

Incumbent will be scheduled based on operational need (rotate shifts, standby, on-call, etc.).

Travel Required

Travel required locally between Excela Health locations.
