Community Care Specialist

As a Community Care Specialist at Service Coordination Inc., you will play a vital role in supporting individuals with developmental disabilities. Your primary objectives include establishing positive working relationships, exploring options for growth opportunities, and securing ongoing support services. You will work collaboratively with individuals, families, and external stakeholders to provide case management services that meet state-mandated standards.
About the Role
This full-time position offers a competitive salary range of $62,217 - $77,160 annually, making it an attractive opportunity for those passionate about human services. As a Service Coordination Advocate, you will be responsible for providing education to individuals and their families, advocating on their behalf, and coordinating services to achieve optimal levels of independence. Key Responsibilities
Some essential duties of this role include:
- Advocating on behalf of individuals and providing resource information to empower them.
- Providing education to individuals and their families on self-determination principles.

- Establishing and monitoring Person-Centered Plans (PCPs) that identify outcomes important to the individual.
Requirements
To be successful in this role, you should have a bachelor's degree from an accredited education program in a human services field or equivalent experience. A strong passion for working with people with developmental disabilities is essential. This position requires excellent communication and interpersonal skills, as well as the ability to work collaboratively with diverse groups of people.
