Job Summary:

PharMerica is seeking a skilled Clinical Pharmacist to join our team. As a key member of our pharmacy team, you will be responsible for evaluating physician medication orders, maintaining patient/resident databases, and ensuring the safe procurement, storage, distribution, and use of drugs and biologicals.

Responsibilities:

• Evaluates physician medication orders for appropriateness of drug, dosage, potential interactions, and route of administration.
• Maintains patient/resident database to include current and past medical history, clinical and laboratory data, and concurrent medication.
• Ensures laboratory tests for monitoring high-risk conditions are ordered, evaluated, and discussed with the healthcare team.
• Collaborates with the healthcare team to provide clinical and technical support as needed.
• Documents all communication with physicians, nurses, and other healthcare team members in the medical record.

Qualifications:

• Bachelor's Degree in Pharmacy; Pharm D preferred.
• One year experience in long-term care.
• Licenses/Certifications: As required by government regulations and business needs.
• Detail oriented; goal oriented, team player, oral communication.
