About the Role

We are seeking a skilled Medical Professional to join our team at Mass General Brigham Home Care. As a vital member of our interdisciplinary team, you will play a key role in delivering high-quality specialized care to patients with chronic diseases.

Responsibilities
  • Provide skilled nursing care to patients with chronic diseases, including assessment, treatment, education, and support
  • Collaborate with interdisciplinary teams to develop and implement individualized care plans
  • Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination
  • Maintain accurate and timely electronic documentation, adhering to organizational standards and policies
Requirements
  • Graduate of an approved School of Nursing
  • Current Massachusetts license as Registered Nurse
  • Minimum one year previous nursing experience in acute care setting, with preference given to those with home care experience
  • Ability to travel within MGB Home Care's geographic area
  • Strong interpersonal and customer service skills, with ability to work with diverse populations
  • Excellent communication and organizational skills, with ability to prioritize tasks and adapt to changing situations
